| To become a member, just go to one of our monthly clean-ups or meetings or submit your information & payment online. We take membership forms & dues at any D5DOG event.
We need some basic information from you when you become a member:
Name and dog(s) name
Street address
Phone number
e-mail address (and if you'd like to get special announcements only or be subscribed to the discussion list)
Parks you use regularly within District 5
Membership level (Basic, Sponsor or Founder) and committee you'd like to be on (perhaps)
And we'd love it if you could also answer these questions for us when joining:
Interested in knowing about other issues that effect San Francisco dogs? (Citywide dog issues, Federal and State Park issues, issues in neighboring counties, health & welfare issues, etc.)
What is your primary goal in supporting D5DOG? (what would you like to see done?)
Why are you joining D5DOG? (off-leash issue, bettering your park, education, health & welfare, fun, community issues)
